Given the way the Manager handled customer service with you, I would not have tipped him a single dong. You are the customer. Him, the girls, everyone working in the establishment is there to provide you a service. Too bad that she had to 'walk back and forth', but it was because she got her period early. So was the cheeky bastard trying to blame that on you? A more customer-oriented Manager (rare in Vietnam but can be trained by not supporting tipping for bad service as an example) would apologize for the inconvenience to you, the customer, at having to switch girls in between due to an unforeseen circumstance. In this case, I would happily agree that he deserves his VND 200k tip. So based on how the events unfolded as you described, I would simply smile, say nothing and offer no tip to him before walking out.

So all in all you spent VND 2.2 M but could have justifiably IMO gotten away with spending VND 1.8 M.
Nationality has very little bearing on a person being a tight ass with his/her contents of the wallet.

BTW, when do I get to see you again???
