As soon as you sit down, the papasan or mamasan will bring a girl over. If you like the girl, keep her. If you don't, just wave them off. You can also walk around and find your girl. This is where your conversational Thai comes in handy. You can also ask for an English-speaking gal, too!
I believe different bars have different prices for the girls to sit with you. I believe the currently rate is between 130 to 150 bahts for a 30-minute block. For example, if you get 6 green receipts at the end of the night, you will need to time that by her 30-minute rate. To put it in perspective, a lady drink in Soi Cowboy or Nana will run you 145 bahts, and you might get 5 to 10 minutes with you before she goes around to chop another carrot.
By the way, the girl I ST'ed spoke little English, but she was super GFE :O)
